    The Wormhead can be seen as a more demonic and corrupted version of the Groaner, as they share a lot of the same physiology and behaviours. Also, their technical name is "Canine Monster (Type B)" which proves that it is more than a theory (the Groaner is also known as Type A). They are encountered in Otherworld, whereas the Groaner is encountered in the Fog World.

    As their name suggests, their heads have been completely overrun by many parasitic worms. It's highly likely that the worms have made the creature even more ravenous than before as their overall muscle density and weight seem to have both increased rather dramatically. Like the Groaner, their heads resemble a large gaping mouth. It's interesting to note that the worms seem to extend all the way down to the neck and then become part of the normal canine body.

    They are sensitive to sounds, having very little or no sight. So whenever they are encountered, loud sounds such as gunfire should be limited and footsteps should be muted and less frequent. If they are startled however, it's quite straightforward to run away if you know where you are headed. If not, and killing one is the only option they will only take a few hits from a Handgun to bring one down.

    Their appearance suggests that Alessa Gillespie's influence over the Otherworld is much stronger than the Fog World, allowing her perceptions to become even more twisted. As such, the Wormhead represents both her dislike for dogs and hatred of writing insects and invertebrates. Also, the fact that the worms could be potentially eating away at the creature could also represent the inevitability of all living things returning to the Earth once decay has started.
