The Split Head is an enormous, hulking beast which has a severely mutated head which acts entirely like a mouth and can quickly open up like a book. When it's mouth is open, strings of thick saliva can be seen. It has a series of small eyes on the sides of the head which are located at the base of two deep recesses - giving it basic vision.
The creature itself is much larger than Harry Mason, and extremely heavy and powerful. However, due to its size and weight it is also extremely slow and takes time to move, particular if it needs to turn around which can leave it at a disadvantage. It's main attack is to catch their prey in their jaws which can either cause major damage, or they can simply swallow their prey which leads to a swift end.
The creatures most likely origin is based on Alessa Gillespie's demonic interpretation of a giant lizard that was part of a children's story book in her school, Middlewich Elementary School, Silent Hill. In the story, a large lizard with a powerful bite fought against a hero that ultimately defeated it by shooting it in the mouth. As was with the storybook version of the creature, the Split Heads weakness was the soft flesh inside its mouth, so shooting it there would damage it most effectively.
The Split Head encountered by Harry Mason is the first boss enemy from Silent Hill, being fought in the Otherworld version of Middlewich Elementary School. The fight itself begins where Harry activates the main boiler. From there, a bound figure (which may represent Alessa herself who was given fourth-degree burns by her mother Dahlia Gillespie) is set ablaze. Then, the creature appears while the figure continues to burn for the duration of the battle. After the battle is over, Harry is returned to the real world.
